My permission is granted for the event coordinator, or an assistant to the event coordinator, or other staff persons in charge of event to obtain necessary medical attention in case of sickness or injury to my child or myself. I also give permission to the hospital selected by the person in charge to treat me/my child with any needed emergency care. I further agree not to hold First Baptist Church of Branson, or any of the accompanying sponsors liable for any accident, injury or death incurred by my child or myself while participating in this event or any events/gatherings sponsored by FBC Branson. I (parent) give my consent for photographs or videos that may include my child and/or myself to be used in FBC Branson publications, website and/or promotional materials.

1) Drinking or possessing alcoholic beverages, even if the participant is of legal drinking age, is prohibited. Smoking is also prohibited. 2) FBCB will provide proper and sufficient supervision by adults at all events. However, FBCB and its employees and sponsors cannot be held responsible for any physical injuries, or loss of personal property that may result from improper conduct. 3) Possession and/or the use of any illegal drugs or the supplying of drugs to any person at the event is strictly prohibited. 4) No gambling is allowed. 5) Vandalism is prohibited and any damage must be paid for by the person who caused such damages. 6) No member may repeatedly distract from planned activities, fail to attend any part of the event, or leave early without the express permission of the advisor in charge. Unauthorized guests will be asked to leave. 7) Any act that is in violation of federal, state, or local laws will be considered a violation of the code of conduct. ANY CODE OF CONDUCT VIOLATION MAY RESULT IN EXPULSION FROM THE EVENT. I, the undersigned, have read these rules and agree that the participant will abide by them.
